Legal Question in Family Law in Illinois

my daughter is 16 she had a baby with a 23 year old man. At the time of the birth the father was in the picture but is no longer,he did sign the purturnity papers. we would like to change the babys last name to my daughters how can we do this.

File a petition in the county where the minor resides, serve the father with notice, appear before the court and plead your case on your hearing date.
