I have a daughter that is 19 years old and is now and as of last Christmas eve is now living with me and my ex wife refuses to terminate child support and unfortunately I'm not sure how to fill out the paperwork for the courts.

Look at the last support order - it may provide for a termination date already. If it does not you either have to go to the courthouse where the order was entered and prepare a motion and notice (which you must send to your ex-wife) to terminate support. The longer you wait the more it will cost you. You could hire an attorney to do it.
