Legal Question in Family Law in Illinois

if the divorce judgment states that the wife has leave to resume her maiden name, do i need to file for a formal request for name change? or can i just go to the Social Security office and get a new card? DMV for a new liscence?

You can just go to the various agencies and request that they change your records to whatever name you are going to be using. You should always take a copy of your divorce decree or Judgment with you.
