how can i enforce a court order that states the non-custodial parent pay the custodial parent for medical insurance

You could bring a motion for contempt and ask the judge to enforce the order.

Petition for Rule to Show Cause. Then you can ask the court to pay your attorney's fees if you prove he willfully refused to pay. (I'm simplifying it a bit).
