Enforcing terms of divorce decree

How do I enforce in my divorce decree that my ex-husband should pay for half of all medical/dental expenses after our insurance pays their portion?

You have to bring him back to court on a post decree matter, essentially a "Motion to Enforce Judgment", or Rule to Show Cause.
