A Guardian ad litem was appointed for my 5 yr old daughter months ago; he spent 5 minutes with her; cancelled appts,ignores my written inquires,made decision for visiting father in enclosed environ but not superv., causing emotional problems for daughter; he will not tell me why won't request supv. visitation

He does not have to tell you anything. He is employed by the court, not the parties. You can bring your concerns to the court, but without proof of wrongdoing, I do not see any issues here.
