Filling in form ccdr0604

I have form ccdr0604 but I am not sure as to whos income do they want to know about just mine or my present spouse together wity mine. He never adopted my children and I file as head of household and my two children. I have court on Feb 15, 2006 HELP I really do not under stand. 708 447-0145 or work 708 327-9149
