Legal Question in Family Law in Illinois

My girlfriend has joint custudy of her 3 children with her ex..she wants to move to Iowa from Illinois. She woould be keeping the same visitation rights to her ex that she has now. And the mileage would not be very different for him. She doesn't have the money for a lawyer and wants to know how to petition the court she she can move. How should she go about doing this, and what forms would she need to supply to the court.
