You have a few options:
1) Call an attorney to file the motion for you. We are happy to volunteer.
2) Call the Court where you will be filing and ask if they have a form.
3) Check online, but make sure that whatever you find is for Illinois and just keep in mind that there is no substitute for a document drafted for your specific circumstances.
