Guardianship or Custody

I have a 11 month old son and also another child who's 11 years old that live with me I recently was told that I have a lump in my breast that the doctors want to try and remove they say its benign but just in case I wanted my sons father to notorize a letter stating that in case of any complications I would want my mother to take over and raise my kids to keep them in the same house so that they can grow up together. Is that possible and what would I need to do?

You need to see a lawyer about this. A notarized letter means nothing. You need a court ordered guardianship for the period you are incapacitated.
