guardianship of a minor
My 16 year old daughter has been invited to move back to Chicago and live with a friend's family (I do know these people and they are suitable). We have been living in CA and I do not want to move back to Chicago. She has had a terrible time adjusting to CA and feels Chicago will help her get better. She suffers from severe anxiety disorder to the point of not leaving the house and Irritable Bowel Syndrome. I do not have medical or dental insurance, and she has CA state health ins. (the mental health system here in CA has done nothing for her) I feel it would be in her best interest to stay in Chicago. The friend's mother asked about obtaining guardianship so that my daughter could be put on her husband's family insurance plan and it would help my daughter get the proper medical care she needs and be in a supportive environment. I travel for my work and the friend's mom is home most of the time. What documents are required? Do I file or does the mother of her friend? Is this done in Illinois where my daughter now resides or here in CA?

The person seeking to be the guardian files the Petition to Appoint Guardian. As long as no one would contest this, it's a fairly easy process.
