Legal Question in Family Law in Illinois

My ex and I both live in IL (and have for 3+ years) but our existing order is from MN. Based on recent events I'd like to file here to change placement and can't afford an attorney (I make decent money but live paycheck to paycheck). I've done some research and know I have to petition to register the judgment from MN but can't find a form. Is there a specific form for this or do I draw one up on my own?

You need to get a certified copy of the judgment. It is not a simple thing to do. Many lawyers do it wrong. There is no form.
