Legal Question in Family Law in Illinois

If my son's father has not seen him his entire life(15 yrs) do I need his consent for his last name to be changed and/or for my husband to adopt him?

Also, what is the procedure and cost for a last name change?

It is difficult to simply change his name. His consent is not needed to adopt, but it makes the matter simpler.
