Spelling/Alias correction for Parent on Birth Certificate

I was wondering if anyone would

know how I would file a court order

or affidavit to correct my mother's

name on my birth certificate.

The hospital used her ''anglicanized''

name (alias?) and mis-spelled her

maiden surname.

You would need to file a motion to correct a birth certificate.
