Sperm Donor Agreement

I'm looking for the correct form in IL for a known sperm donor to give up his parental and legal rights. I can't seem to find a form labeled as such. Please advise what the form for IL is. Thank you.

I am not aware of such a form. If you are working with an agency it can be obtained through the agency. If not I would advised you to seek a legal consultation with an adoption lawyer who can prepare a document to protect your rights.
