Visitation and child's health insurance

My wife and I divorced in 2004. I have my daughter from Saturday morning to Sunday evening every week plus two weekdays every week. Is my ex wife responsible for driving half the time? I do all of the driving. Also, I have health insurance for my daughter but my ex refuses to take her to a doctor that is covered by the plan, so nothing is covered and she expects me to pay half of the bill. Should she have to take the child to a doctor covered by the plan or am I required to let her do this?

As an attorney, I would certainly argue that she must take the child to a covered doctor if possible. The easy way to solve both questions is simply to file a post decree petition to modify the decree to force her to drive 1/2 of the time and require her to use covered doctors. That is the only way you will be sure to have her follow the rules.
