voluntary transfer of custody of my child

I need to transfer custody of my daughter, to my brother in Il, for safe keeping. I need help with what document and legal requirements I need to have this done. My brother needs to have gaurdianship of my daughter, as well as financial responsibilty, and the ability to make all decisions on my behalf. The document must be valid in both MN, where I live, and IL, where he lives. The document must be valid by the school system, health insurance, and the courts. Please let me know what I need to do, or have the document say, in order to have this done. thank you--name removed--

If this is a Minnesota matter, you would need a Stipulation and Order granting the family member custody. This would require an agreement of both parents.

If this is a temporary matter, you could accomplish the same thing with a Power of Attorney. However, the other parent could negate the POA and seek custody of the child.
