Write and Prepare a Legal Proof

Can you tell me how to write a legal proof for reimbursement of daycare expenses? Can you tell me where I can find an example of a legal proof?

There is no standard form. Suggest you use an affidavit form, describing the transactions, attaching copies of receipts or canceled checks, sign your name before a notary. Form of the proof should depend on the reason and purpose for which it is being submitted.
