Form for Request for Change of Address or Boxholder
I need to obtain a copy of the form "Request for Change of Address or Boxholder. Information needed for Service of Legal Process." I filed a suit pro se to collect a deposit from a landscaper who never performed any work according to our contract. When the sheriff attempted to serve him, the sheriff was told the landscaper doesn't live there any more. The post office in that town says I need this "Request for Change of Address or Boxholder" to see if a forwarding address exists. Thanks!

Check with the Post Office
If this is the form that I think you're talking about, it should be available from the Post Office. You file the form with a fee, which used to be $1.00 (and may still be). If this is not the form, then, I'm not sure what you're asking about.
Post office form
The post office should give you a the form. You may need to have a courtdocument showing that you have filed suit and are representing yourselfto get the form. The sheriff may be able to obtain this form if youcannot. The Post Office Manual has a copy of the form in it. The postoffice has imposed restrictions to prevent abusive spouses and criminalsfrom getting addresses. Therefore, I am reluctant to post this on theweb.
