Miscellaneous remedy of also known as

My wife needs to file a miscellaneous remedy for her name. She needs to petition an Also Known As to be attached to our marriage certificate. The County Circuit Clerk was not helpful as to how to do this. I need help, perhaps a sample miscellaneous remedy.
