Legal Question in Landlord & Tenant Law in Illinois

Which form should I use?

I have a tenant in Illinois that will be coming off of a 1 year lease in the next week and will need to be converted to a month-month rental agreement.

What form may I use that both states that all prior ''terms'' still apply however the only difference is the length of the agreement?

Just enter into a separate written agreement with the terms that you stated.
