Legal Question in Personal Injury in Illinois
I need an example form to give my employer to sign for loss of time at work due to an accident where I was injured. I need it for a court case and I don't know what it's called. I have an attorney but he did not give me anything? I was going to type one up myself but I want it to be very professional. Please help?

Have your attorney get it for you.
If you have an attorney, you should speak with him. He knows your case and may not want just any "form". If he wont provide you with a form or make the request himself, you might want to find another attorney as usually this is soemthing an attorney obtains.
Aside from that which has been suggested by others, perhaps there is a good reason why your attorney is not providing you with one. Perhaps you should talk to him and find out why he is not doing what you believe he should.
