Call or email and ask them to verify whether you are receiving your Homestead Exemption or not. If not, have them mail/email/fax the form so you can fill out the document and receive it going forward.

If you were entitled to the Homestead Exemption for past years, but didn't receive it, ask the Assessor if they will issue a "Certificate of Error" to give you the benefit of that Exemption by way of a refund check for a portion of the property tax that you overpaid as a result of not having the exemption.
