My wife and I divorced in July 2011 and we're still on good speaking terms. At the point of divorce, we left my name on the mortgage - can we have my name removed without going through a refinance?

You may approach the lender, but there would not be any means to compel them to modify the terms. They may ask your wife to requalify the same as she may in a refinance. The property value may come into play. The mortgage holder made the loan on the basis of both of you qualifying and they are not compelled to take on more risk by only having one of you on the hook.
