As a church treasurer, I found the minister filed the income tax report with an unauthorized W-2 form for a few years. What would be a right action I should take?

I am not sure what "income tax report" to which you are referring. Do you mean the minister's personal income tax return or the church's Form 990? Assuming you are referring to the church's form, as treasurer you should consult with an attorney licensed in your state or consult with the church's accountant to consider if the church's report should be amended.
