attorney representation court filing

Does an attorney file a specific form with the court when he is hired by the client for representation in a case? If so what is this form called? Where can I access form.

The question is for illinois county appellate court

There would only be something filed in the court when a lawsuit is filed, and that would be the complaint or petition. There is no "form" that is filed when an attorney represents an individual.
