Name of document

In the event a person becomes fatally ill, or in an accident of some sort that would require heroic measures, or life support, what is the document called that you can prepare to ask that these measures not be taken?

It's called a *LIVING WILL* and it applies only if you are 1) terminally ill 2) with no hope of recovery, and 3) death is imminent -- the care provider is then to withdraw or withhold death delaying measures.

There is also a "Durable Power of Attorney for Health Care" which gives your named agent the power to make a broad variety of health care decisions for you while you are incapacitated.
