how do i obtain power of attorney over somone who is incapacitated

If that person lacks the mental capacity to manage his affairs, you cannot simply use a power of attorney. Instead, you must file a Petition to become that person's Guardian. Feel free to call me if you'd like to discuss this further. I won't charge for the phone call.
