Power of attorney

I am looking to appoint my life partner as my ''attorney in fact'', but i am confused with the various types of power of attorney forms that are available, i.e. power of attorney for health care, durable power of attorney, and power of attorney for mental health, and a living will. I want my life partner to have ALL of the rights listed in these documents, but is there any one form that covers ALL these rights without us having to fill out 4 seperate documents?

its a little more complicated than that.

The forms are just tools. You could probably combine them and put them on one form.

The real question is does the form accomplish all of your objectives and plan for all contingencies

What you really need is a comprehenisve plan.
