im not sure what kind of information you need.but here is what

happened.I was injured on my job the 12/28/04was sent to the company doctor

which he had thought i had a pulled muscle so the company doctor sent me

through thearpy for 4 or 6 wks 3 days a wk put me on light duty.which in

the mean while i was put on a machine cause a job had to be done that

day.I told the floor lady plus the supervisor and the owner that it was

hurting my arm to be working on the machine i was told that i had to

stay there until the job was done cause it had to go today and the owner

told me as soon as the job was done that he would put me back in hand

work which was light duty.my elbow was so sore from working it like i

did i called into work the next day plus missed thearpy.I went to thearpy

the next day and told them why i had missed my appointment. i seen that

doctor last 1/27/05 he sent me to an orthopedics surgen and was seen by

her 1/22/05she then took xrays and said i had tennis elbow plu!

s tendious in my wrist and thought a cordazone shot in the elbow would

help so i got the shot and she is now haveing me go through 3 wks 3days

of thearpy which the doctor said most need surgry but she said it was
