If an attorney submits a letter to the insurance carrier to pay an overdue bill with a request for a written answer if the bill cannot or will not be paid and the insurance company does not do either. What is the next step for remedy?

It all depends on the context.
Usually, if you do not agree with your insurance about the decision they have taken, you can take the matter to an arbitration (read the policy to see what is the remedy) and have the issue resolved that way.
You can also contact the department of insurance and have them inquire about the issue and see how they answer.
The insurance laws require an insurance company to evaluate a claim with due diligence and without any undue delay. If there is evidence of bad faith, you are entitle to sue the insurance company and collect fees, fines and other damages.
