Usually in a personal injury case, a medical provider may file a lien to protect his medical bill. The lien will be payable when the case is resolved and there is money to be distributed.

Usually, a lien against a lawsuit means that the lien holder claims an interest in the money produced by the lawsuit. If there is no money, there is no interest. So the only way the lien becomes a real interest is when there is money and that usually happens at the end of the lawsuit.
