Legal Question in Workers Comp in Illinois

Release of medical information

I was injured on the job. I am told I will recover. The workmans comp company sent me a Authorization for Medical Information to sign. I don't like the fact it seems to last forever. The accident is not mentioned. It basiclly give up all my privacy to anyone the insurance company wants to tell. I have nothing to hide I just don't like the way it is worded. Do I have to sign it?
