It's 3 years from the date of the accident or two years from the last payment of compensation. But for multiple reasons, which I would be happy to discuss with you, you are better off filing formally sooner rather than later.

For a "third party" claim against a negligent party you generally have two years to file a lawsuit.
If that negligent party is a City, the State or the CTA (for example) the rules are different.
To file a claim in the Illinois Workers Compensation Commission, you have three years from the date of the accident or two years from the last payment of compensation assuming you gave your employer proper notice of the accident.
