How to write a General Diary with Police for loss of ration card

Dear Sir / Madam, (Jalpaiguri, West Bengal)
All you need to do is write a simple letter addressed to the Sub-Inspector at the Police Station in whose jurisdiction your place of residence is, get an acknowledgement and apply for a new ration card by submitting a Court affidavit to complete the process, as advised by the concerned Rationing Officer in Jalpaiguri.
