format for succession certificate

i am vivek pandey ,my passed away in the yr 2002, a few days back i got know about a investment he had,he had invested rs 10000 for two bonds of icici safety bonds ,redemption date was 24.06.2003.

i want a format to be prepared by me to get the redemption amount,in

case the claiment would be my mother.

Succession Certificate is issued by the Court so what exactly do you need.

find out from ICICI Bank Ltd. what type of indemnity they require. If they insist upon Succession Certificate, apply through lawyer in the court within whose local jurisdiction the deceased bondholder permanently resided.
