Change in Surname

After filing an affidavit for surname change what about the changes to be made in PPF a/c , LIC, Bank, Passport & other important doucments. What are the procedures for updating the records?

PPf, LIC etc., may not accept just an affidavit! pl make a gazatte and papaer publication and produce the same along with the application for change in the policy ..

contact the concerned department and ask them to issue new document with the altered name.

good morning

make an affidavit relating your new surname and the old surname. the purpose of the affidavit should mean the identity of both the surnames is same and thats you and thats all for the procedure

You have to fill up necessary forms and also to complete other formalities for change of surname in PPF etc.Consult agent of Insurance and officials of PPF.
