You may first need to change your name in your educational records by following the prescribed procedure which may include deposit of fee with the concerned institution and also a publice notice in the news papers of your area. For appropriate guidance, you may contact the educational authorities of your area. On the basis of change made in your educational records / certificates, you may approach other authorities like PASSPORT, PAN CARD, ETC.
