My full name is VIVEK without any last name . I want to add KUMAR as my last name as i face difficulty to fill applications online which require mandatory last name field.
Is it change of name?
Kindly tell me the legal procedure to get it done. My permanent address is Bihar(india)

you can make an affidavit and get it notorized and there after publish the change of name in a local language newspaper and an english paper
