certificate case of bank loan

bank filed a certificate case against his principle debtor in the court of exicutive megistrate and issued a notice to p.d.but now how the p.d. appears with petition.we want to know about performa of the petition which is filed before the court of executive megistrate.

There is no proforma of a petition. You need to file a statement of your facts; and if you admit your liability towards the Bank, be prepared with a repayment plan.
