For changing your name, you need to submit the information in writing relating to change of name in an affidavit format to the Special Executive Magistrate who would attest it based on proof of identity. After that a public notice in the newspaper about such change is to be given. In mumbai, you need to notify the same in an official gazette to complete the process of public notice.
