Re: Change of religion
There is no law in India which governs religion. India is a 'secular' country having no religion of 'State". You are free to practice any religion.
A muslim who starts faith in Hindu way of life becomes a Hindu.
for your own satisfaction you may change your name and there is a legal procedure for that - making a declaration in a local news paper, getting it changed in your school records and other records.
you may contact a local priest for formal conversion, if you so like. The VHP keeps organizing camps for conversion. You may contact them.
Re: Change of religion
No law prescribes any procedure.

you can approach any Pandit ji of Arya Samaj Mandir who can perfrom some ritual which is called shudhikaran etc. and will issue a certificate that now onwards you are converted to Hindu.
