Maiden name is your personal choice. However, after remarriage, you must change the name of your spouse in the passport.

Apply for baby passport with name of his step father.

you may change your surname without any problem but baby's name cannot be changed to his step father's name as his real father, the natural guardian is still alive. either you seek his written consent to this effect before the court or you can use only your name as guardian/parent of the child.

There is no law requiring you to change your name; the child's last name may be changed only if your ex-husband consents for adoption by your second husband.

all of the above are correct.

It is your wish whether you want to change your surname as your parental surname or present surname or new husband surname. Regarding child you cannot change till the consent of your exhusband.
