we want to apply for the mutual divorce, could you plz tell us what is d formalities and also where do we get the application forms

There are no application forms. You may need to draft an appropriate petition under the H.M.A. in accordance with law and present before the District Judge of your area. It may be better to engage a local lawyer. You should also be careful about the terms and conditions as to custody of children,if any, and maintenance / permanent alimony etc.
There is form for the same. You have to get a proper suit filed through a Lawyer who will further guide you.
