passport size photographs, residence proof, date of birth proof of both, marriage photographs, witnesses

If you are Muslim, Nikahnama has to be filled in the presence of parties and witnesses before Nikah Registrar, otherwise act on the advice of Mr. Rajiv Gupta.

You may need to provide proof of marriage in the share of duly sworn in affidavits of the parties and witnesses, including a priest who ceremonised the marriage.

Depends and varies from state to state but ordinarily pp size photos of both, 2 to 3witnesses, age proof, marriage proof etc.
