in india divorce has to be filed giving details of marriage, and grounds of divorce. There is no form as such. Some states do give guidelines in their High court. You mention the state in which you got married and then a link may be sent. But, it may not be practically possible for you to manage prosecuting a divorce matter unless it is no fault that is mutual consent divorce.
