reliance is the dependance of party on certain fact of evidence or document

address form may be address memo stating address of the party to the court

process fee is the fees charged by court to serve process

summons is the direction of court to remain present before court on particular date and timings when matter is fixed. The summons is served well in advance before such fixed date

They are documents to be attached with the plaint, at the time of filing of suit. Except summons, summons is a intimation to defendant to appear in the court on fix day and time
