addition of surname to the passport

i have my indian passport already issued to me. i want to add my surname to my passport. i have all my educational and other documents in my and my parents' first name only. what formalities i have to do to get my surname added to my passport ? pls guide.

You may visit passport.gov.in.

You can download the forms and file an affidavit.

OR

You may visit the Regional Passport Office.
