It is very simple. In the Southern District of Indiana ( I can't speak for the Northern District), you simply file a Notice of Conversion to Chapter 7; shortly after that you will receive an Order from the Court directing you to file certain information updating the case.
Of course, if you have had a prior bankruptcy, your ability to convert may be limited. Talk to your attorney.
