Legal Question in Business Law in Indiana
This isn't really a legal advice question, but I am starting a business and I want an attorney to write a wavier for my business and fill out the forms for me to start a LLC. How much would this cost?

Each law office has a different fee structure. Some charge hourly and others charge per project on a flat fee basis. $1550 would not be unreasonable.
